When to Use This Tool:
Use the Barcode Generator when:
Preparing printed cover sheets or transmittal documents for statutory filings external to Gain
State regulators require physical barcodes on paper submissions
You need to generate barcodes for multiple filing variations (original, amended, quarterly)
Note: Not all states require barcoded paper submissions. Check your state regulator's requirements before generating barcodes.
How to Generate a Barcode:
Step 1: Fill out the Form
Enter your NAIC Company Code (5 digits)
Enter the Filing Year (4 digits)
Search or type the Document Identifier (3 digits)
Start typing to filter document types
Example: Type "201" or "Annual Statement"
Select the State Code (2 digits)
Select Data Indicator, Filing Type, and Filing Period from dropdowns
Step 2: Generate
Click Generate Barcode. The barcode preview appears on the right side of the screen.
Step 3: Download
Click Download PNG to save the barcode image to your computer.
Step 4: Place in PDF
Place in the upper right-hand corner of the pages and forms as identified on the NAIC Document Identifier Codes listing
Frequently Asked Questions
Do I need to generate a barcode for every filing? NAIC PDF filings do not require barcodes, however, it is okay to include them in the ZIP file submission. Some states pull information from the NAIC website and need the barcode affixed to certain schedules. Gain automatically includes barcodes on all applicable PDFs generated from our software. Check your state requirements for externally created filings and supplements.
Can I use this for electronic-only submissions? Barcodes are typically not required for purely electronic submissions. However, some states may require barcoded cover sheets even for electronic filings. Check your state requirements.
What's the difference between NAIC and State-Specific filings?
NAIC (0): Standard NAIC-mandated forms filed in all states
State-Specific (1): Custom forms required by individual states only
State codes identify whether a document applies to a specific state or jurisdiction. Use the two-digit NAIC Schedule T code:
00 for non-state-specific forms (most common)
01–51 for individual U.S. states (e.g., 03 = Arizona, 33 = New York)
58 for "Other Alien" jurisdictions
59 for "Grand Total" - use this when filing Supplemental Exhibits and Schedules Interrogatories with no state-specific data to report
